The Behavior Specialist (BS) provides community-based therapy support services. Adults and children with developmental disabilities are the primary population supported by the Behavior Specialist. The Behavior Specialist provides a functional behavioral assessment along with the development of a behavior support plan of care and ensures the implementation and monitoring of that plan across settings. As a member of the person-centered team, the Behavior Specialist will utilize the most current evidenced-based strategies available in applied behavior analysis.
MPW, SCL Waiver
The Person- Centered Coach (PCC) provides community-based support services. Adults with developmental disabilities are the primary population supported by the Person-Centered Coach. The Person- Centered Coach in conjunction with the Behavior Specialist and Case Manager ensure the proper implementation of person centered support plans.
SCL Waiver
The Community Access Specialist will provide support and training to a waiver participant that enables that participant to develop a network of natural supports to achieve a clearly defined and valued social role within the participant’s community.
SCL Waiver
Community Living Supports shall facilitate independence and promote integration into the community for a participant residing in his or her own home or in his or her family’s home.
MPW Waiver
The Personal Assistant shall enable a participant to accomplish tasks that the participant normally would do for himself or herself if the participant did not have a disability. The Personal Assistant shall be available only to a participant who lives in the participant’s
own residence or in the participant’s family residence.
MPW, SCL Waiver
A respite care service shall be short term care based on the absence or need for relief of the primary caretaker.
MPW, SCL Waiver
The Supported Employment specialist shall be services that enable a participant to engage in paid work that occurs in an integrated community setting with competitive wages and benefits commensurate to the job responsibilities.

We are accepting referrals for Michelle P. Waiver (MPW) and Supports for Community Living (SCL) waiver participants. We also serve individuals who receive State General Funds/ PASRR.
